We often explain Cialis using a simple analogy. Erections depend on good blood flow. ED happens when blood flow is reduced or when the signals between brain, nerves, blood vessels, and hormones are not working smoothly.
Here is what happens when you take tadalafil:
Sexual stimulation starts the process
You still need sexual arousal. Cialis does not create desire by itself. When you feel aroused, nerves in the penis release nitric oxide.
Nitric oxide improves blood flow
Nitric oxide signals blood vessels in the penis to relax so more blood can flow in. This creates and maintains an erection.
Cialis supports that signal

As-needed dosing
- Typical starting dose: 10 mg taken before sexual activity
- Can be adjusted to 5 mg or 20 mg depending on response and side effects
- Take at least 30 minutes before sex
- Do not take more than once in 24 hours
This option works well for men who:
- Have sex less frequently
- Prefer only taking medication around those times
- Want flexibility to adjust dose up or down if needed
Daily low-dose dosing
- Typical dose: 2.5 mg or 5 mg once daily
- Taken around the same time each day
- After several days of steady dosing, erections become more reliable whenever arousal happens
Daily Cialis can be ideal for men who:
- Have sex more frequently or spontaneously
- Do not want to plan around a pill
- Have both ED and urinary symptoms from BPH

Who Is a Good Candidate for Cialis

Good candidates often:
- Have trouble getting or keeping an erection firm enough for sex
- Feel frustrated with inconsistent performance or “losing it” mid-encounter
- Have cleared heart health with a provider if they have risk factors
- Are not taking nitrate medications or certain heart drugs
- Are open to honest conversation about sexual function
Conditions we screen carefully for
- Unstable heart disease or chest pain (angina)
- History of stroke or heart attack
- Very low blood pressure
- Severe liver or kidney disease
- Certain eye conditions (like retinitis pigmentosa)
- Use of nitrates (e.g., nitroglycerin) or “poppers” (amyl nitrite)

Alcohol
Moderate drinking is usually safe for many men on Cialis. Heavy drinking can:

Food
